We are seeking of a full time Care Manager in our HOPE Program (Helping Offenders on Probation Excel) at the YSM Evergreen Centre for Street Youth.  The Care Manager will work in conjunction with the Hope Program Coordinator to support criminal-justice-involved street youth in their personal growth and development while going through the HOPE program. This will entail supporting them to access and benefit from an array of programs at YSM’s Evergreen Centre for Street Youth at 365 Spadina Ave. including Health and Dental care, Mental Health supports, Food, Housing, and Employment supports to help youth achieve their goal to meet all probationary requirements and break the cycle of recidivism and poverty.

The Hope Care Management role is a front line position and will report to the HOPE Program Coordinator. The HOPE Program is accountable to YSM’s Director of Street Involved Services for outcomes, best-practice and the achievement of YSM’s purpose.


The successful candidate will be responsible for: 

  • Recording case notes and tracking individual progress on a software platform (TIMES) with case notes and touchpoints; along with supporting and managing referrals through our continuum of care model
  • Facilitating case management meetings with individual youth and other program services
  • Accompanying youth to court dates, detention centre visits, family interventions, housing meetings and other appointments as required
  • Maintaining a safe space for diverse participants, including, racialized individuals, people of any religious affiliation, the LGBTQ2S community and people living with mental illness and varying abilities
  • Helping out the Evergreen Centre Drop-In staff when called upon.
  • Providing effective intervention (de-escalation) in crisis situations
  • Building meaningful relationships through outreach with youth who access YSM’s Evergreen Centre’s services and challenging them to seek their full potential
  • Active participation in the greater YSM and Evergreen team meetings and initiatives that address ongoing program development, cross service interface and evaluation 


The successful candidate will have the following skills and training: 

  • Experience working with marginalized people (knowledge of the black youth(16-29) experience in the criminal justice system, courts and police services a definite asset
  • Post-secondary education 
  • Demonstrated knowledge in working with street-involved/at-risk youth from culturally diverse backgrounds
  • Thrives working within a multi-faith, multicultural, anti-oppressive framework
  • Demonstrated excellent interpersonal skills
  • Computer/Keyboarding skills and administrative/organizational skills 
  • Ability to manage crisis situations
  • Strong conflict resolution skills 
  • A Vulnerable Sector Police Check will be required/First Aid and CPI is an asset 
  • Demonstrated personal assent to YSM’s principles of faith, mission and values. Able to sign YSM’s Statement of Faith annually
  • Able to work flexible hours and/or days
