Ray of Hope is currently seeking a Case Management Coordinator to join our team at our Residential Treatment facility. 

Overview

The Case Management Coordinator (CMC) works alongside the Residential Program Manager in overseeing and managing treatment planning for participants of the residential program, with the primary focus on all aspects of the intake, pre-admission and treatment planning processes. The incumbent will support, guide and direct staff in the treatment of, and may also include some direct clinical services for clients of the residential program. They may participate in staff orientation, on-going training and performance management under the direction of the Program Manager and Director. The CMC, in consultation with the program manager and director may provide direct support to participants’ family members by means of individual and family counselling/psychotherapy and mediation and may provide case management for participants by arranging for necessary supplementary services. The incumbent will facilitate communications with participants’ other professional supporters. The CMC provides clinical supervision within their assigned program area and in other areas of Youth Support Services as needed.

Specific Responsibilities

Intake

  • Respond to requests for information (first contacts, referrals) within 48 hours
  • Conduct pre-admission screening and assessment interviews in a way that is respectful to the prospective client’s stage of change and is trauma informed
  • Motivational Interviewing and conveying hope are core responsibilities of the role
  • Provide initial support to families and youth seeking treatment while they move through the pre-admission process
  • Manage complex and volatile interactions between youth and their caregivers with sensitivity to privacy, power dynamics and the need for support, respect and care by all members of the family system
  • Maintain clear communication with other agencies and service providers involved with youth
  • Monitor prospective participants while they are on the waiting list
  • Provide brief counselling where appropriate
  • Fully document the pre-admission process
  • Make recommendations to the Program Manager and Program Director regarding appropriate candidates for admission, and suggestions for treatment planning
  • Make referrals to other community resources as needed

Clinical

  • May provide clinical supervision to staff in consultation with the Program Manager and Director, aimed at developing staff capacity as trauma capable Youth Support Workers in a therapeutic community milieu
  • Required to make decisions and provide team leadership around issues such as: risk assessment & duty to report; assessment & treatment planning
  • Required to communicate decisions to staff, delegate work, and be responsible for ensuring work is done in a timely, competent manner
  • Will ensure parent coaching and counseling for each youth in the program is offered and options are provided for follow through
  • In conjunction with Program Manager, provide individual treatment to participants (e.g., step work, crisis de-escalation, mediation)
  • Take the lead in treatment planning for youth in residential treatment
  • Provide case management for participants, maintaining good working relationships with participants’ other professional supporters, and making well-informed referrals to other necessary services
  • Participate actively in the student placement selection process and provide administrative & clinical supervision to students
  • Maintain membership in good standing with the appropriate College of registered health or social work professionals
  • May be required to work evenings & weekends
  • Considerable risk for vicarious trauma; mild risk for physical and/or emotional abuse by clients
  • Required to have daily access to a road-worthy vehicle and carry insurance with two million dollars of liability coverage. May be required to transport youth, alone in their personal vehicle at times

Administration

  • Ensure professional standards are maintained with respect to documentation, medication management, safe food handling, as well as other occupational health and safety expectations
  • Participate in weekly case management/staff meetings with the program manager and director
  • Actively participate in the planning and implementation of staff training activities.
  • Respond to staff and youth requests and crises in a responsible, timely and flexible manner, in consultation with the program manager/director; and ensure the development and implementation of appropriate, person-centred responses occur.
  • Participate in the on-call manager rotation
  • May be required to prepare and present public education and prevention presentations related to substance abuse for youth, families and other professionals
  • May be required to attend public, educational or Ministry meetings as assigned

Professional Development

  • Participate in on-going professional development activities and engage in reciprocal relationships of learning with colleagues to stay current in the field
  • Participate in YSS Manager meetings
  • Implement related learning and professional development plans as discussed with program manager/director
  • Maintain professional credentialing, certification and continuing education requirements, on own initiative and in conjunction with organizational resources where available

Qualifications

Education

  • Master's Degree in a field relevant to the practice of psychotherapy (social work, counselling, nursing) with specific training in psychotherapy and 5 years of experience; 

OR

  • Undergraduate degree in a field relevant to the practice of psychotherapy plus 5 years of experience in psychotherapy

Experience

  • Addiction treatment, both harm reduction & abstinence-based approaches: 5 years
  • Individual, family and group treatment: 5 years
  • Experience providing treatment in a therapeutic community: 2 years
  • Clinical supervision: 3 years - 5 years without a Masters’ degree

Certifications

  • Standard First Aid and CPR Level C +AED
  • Crisis intervention technique training NVCI
  • Safe Food Handling is an asset

Knowledge/Skills/Abilities to be successful in this position

  • Minimum requirements - Word, Excel, PowerPoint, DATIS

Requirements

  • Vulnerable Sector Check/Police Check
  • Valid, unrestricted Class “G” Driver’s licence
  • Ministry of Transportation Drivers Abstract
  • Pre-Employment Health Assessment Form including immunization record and fitness assessment
  • Maintain a membership in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) or College of Registered Psychotherapists of Ontario (CRPO) or another equivalent professional association (as approved by Ray of Hope) that grants you access to the Controlled Act of Psychotherapy.

Physical Demands

  • Constant standing (25%), sitting (50%) and walking (25%)
  • Often required to ascend or descend stairs.
  • Rarely required to stoop, kneel, reach above shoulders
  • Sometimes required to reach below shoulders
  • Frequent use of mental capabilities and concentration
  • Possible exposure to hazards such as harassment, physical or verbal aggression or aggressive behaviours, witness to harassment or violence, and similar variables. Must have the skill to verbally de-escalate crises and to practice basic self-defence in an unpredictable environment
  • Possible risk of vicarious trauma
  • Infrequent exposure to environmental hazards, including Hot/Cold and regular environmental variables
  • This position requires the person to consistently work with and around others
  • This position requires on-call availability
